• Space and Area to grow out mushrooms.
• Valid ID (must be a resident and registered voter of Calamba City)
• Request letter of Individual Entrepreneur / Grower or organization group / Cooperatives (Address to the Mayor to Sir Opulencia.)
• Fill out profile form with 2x2 picture attached.
• Attend Orientation/Seminar/Training.
• Passed the inspection of Livelihood Division.
